Service offered: Information, advice and support for carers, including Young Carers service (8-18 year olds) and specific worker for Young Adult Carers (16-35 year olds). Also 2 services for vulnerable adults: 1. Home from Hospital service for anyone over 18 returning from a visit to hospital and needs short term, low level support. 2. My Neighbourhood – a wellbeing and prevention service – offering support to anyone over 18 needing help due to social isolation
Target group: Unpaid, family Carers from 8 years old, looking after someone who is ill, disabled, elderly; or suffering from mental health problems or drug / alcohol dependency. Vulnerable adults.
