Service offered: Information, advice and advocacy services, Friendship Support Scheme and telephone befriending, Support for people on Direct Payments and Personal Health Budgets, GP support Pilot within the South East Locality of the borough, Carers’ Support (respite support within the home) and Help at Home (practical support around the home), Pathways for Life, a service funded by the Local Authority and CCG, which provides a variety of support for people with dementia, their carers and family in both the community and Manor Hospital

Target group: Older people (aged 50+) including those with disabilities and/or mental health problems.
