Service offered: Housing advice, support and practical assistance for vulnerable people to maintain their health and independence and be warm, safe and supported in their homes. In Stoke on Trent we can offer low cost handyman service that help with DIY jobs and free safety and security measures. The hospital discharge service supports patients to return home after a stay in hospital through small adaptations, key safes, emotional wellbeing service and giving practical support with managing money and essential repairs. We offer help with improvements and adaptations, specifying and overseeing home improvements, repairs, and funding options across all the areas we cover
