General information
Service offered: Advice and support for all non-medical issues patients of BCHC face. Patients will receive a regular call from a social prescriber to listen to what matters to them on whatever non-medical issue they would like to talk about, from where the SP will signpost out to further advice services or community groups that could help the patient. SPs have contacting with the council, local charities and local community groups, targeting amongst other things loneliness, activity/fitness, finance, housing, benefits, mobility and more.

How to contact: Email occg.socialprescribing@nhs.net with a name, date of birth and issue that would like to be discussed. A SP will then call to arrange an initial appointment. Clients can also call our public number 01295 256261, however should be advised that lines are always very busy and thus waiting will be required.
