Service offered: Work in partnership with Suffolk Social Care Services to support young people aged 16 - 21 (up to 24 if in full time education) during the transition from care to independence. Also offer support with finding accommodation, advice on benefits, education, health issues etc.

Target group: Young people who are or have been looked after by Social Services.
