Service offered: Practical help, befriending and support for patients and carers who are looking after people at home with a life-limiting illness. Befriending and sitting service allowing carers to take a break. Healthcare assistants provide personal care to patients at the end stage of their illness. Grants and a transport service. Only accept referrals from health or social work professionals.
Target group: People living with terminal illness and bereavement, and their families and carers.
