General information
Service offered: Conwy and Denbighshire Stroke Recovery Service provides information, advice and emotional support following a stroke. Whether you are a stroke survivor, carer or family member, we will work with you to identify and address your needs and support you to rebuild your life after stroke. This service offers: Coordinated support throughout your stroke journey. Home visits, telephone calls and / or online meetings. Leaflets, factsheets and advice about benefits. Guidance if returning to work. Assistance with accessing social activities and self-management tools. Support for carers. Signposting to other organisations that could help.
Target group: This service is for stroke survivors, families and carers of people affected by stroke, living in Conwy and Denbighshire, including Colwyn Bay, Rhyl, Prestatyn, Llandudno, Abergele, Denbigh, Ruthin, Conwy, Llangollen and St Asaph
