Service offered: This service offers: Coordinated support for up to 12 months after stroke. Home visits, telephone calls and / or online meetings. Information leaflets and factsheets. Advice about life after stroke inc benefits, driving and returning to work. Assistance to access face to face and online peer support and social activities. Support for carers and family members. Referrals and signposting to other organisations that could help.
Target group: This service is for stroke survivors, families and carers of people affected by stroke, living in Conwy and Denbighshire, including Colwyn Bay, Rhyl, Prestatyn, Llandudno, Abergele, Denbigh, Ruthin, Conwy, Llangollen and St Asaph
