General information

Service offered: This service offers: Coordinated support for up to 12 months after stroke. Home visits, telephone calls and / or online meetings. Leaflets, factsheets and advice about benefits. Guidance if returning to work. Assistance with accessing social activities and self-management tools. Support for carers. Signposting to other organisations that could help.

Target group: This service is for stroke survivors, families and carers of people affected by stroke, living in Conwy and Denbighshire, including Colwyn Bay, Rhyl, Prestatyn, Llandudno, Abergele, Denbigh, Ruthin, Conwy, Llangollen and St Asaph
