General information
Service offered: Adult Hospice @ Home provides specialist palliative care, end of life care and symptom control in the patient’s own home. Care Home Support Team provides specialist palliative care support for care home staff patients and their families. Living Well provides 12-week goal-focused programme for people living with life-limiting illness. Wellbeing Service provides drop-in access to a wide range of therapies, groups and classes, counselling, chaplaincy, family support, bereavement support, and carer support. Inpatient Ward (Adults and Young People over 14 years ) provides Specialist palliative care, end of life care, symptom control, respite for carer crisis and booked respite. Children’s Community Service provides specialist palliative care, end of life care, symptom control, chemotherapy, short breaks in the child’s own home or school, events, outings and Friday fun clubs, music therapy and play therapy
Target group: Adults with life limiting illnesses and children with life threatening and life limiting illness, and families and carers of these adults and children.
Area served: Adults - Dartford, Gravesham, Swanley. Children - Dartford, Gravesham, Swanley, West Kent and Bexley.
