Service offered: palliative care delivered by clinical nurse specialists. Hospice at home support for patients in the last 3 months of their lives for additional care to support patients and families. Day care. Psychosocial support from counsellors and social worker. Bereavement support to relatives.
Target group: People with advanced terminal illness.
