Service offered: Palliative care delivered by clinical nurse specialists on the Hospice ward. Hospice at home support for patients in the last 3 months of their lives for additional care to support patients and families. Day care. Psychosocial support from counsellors and social worker. Bereavement support to relatives. Wellbeing support through St Raphael’s Wellbeing Centre.
Target group: People living in Merton and Sutton with a life-limiting illness and their families.
