General information
Service offered: This service supports clients who are suffering from acute and chronic illness who have recently been discharged from hospital. They provide support with settling people back into their homes, making sure that there is food, all facitilies are working, medication prompts and other practical tasks. Referrals will be made for all welfare benefit checks and support. The support is available for up to 2 weeks after discharge.
Target group: Anyone aged 50+ who has recently been in hospital
