General information
Service offered: Support and practical help for patients aged 50+ upon discharge from hospital, for up to 6 weeks. Advice on various subjects including welfare benefits and community care.This service support clients who are suffering from acute and chronic illness who have recently been in hospital. They provide support with welfare benefits, shopping and other practical tasks and monitoring for up to 6 weeks after discharge.
Target group: Anyone aged 50+ who has recently been in hospital
