Service offered: Advice and information for older people and their carers on community care, housing and welfare benefits. Community based support and liason with other health care providers, especially GP’s. Support in the home with a range of services including med prompting and preparing meals and cleaning. Assisted bathing. Day time activities and Lunch Clubs. Transport to hospital appointments . Dementia care. Carers support including a sitting service. Shopping Home from hospital support. Support to remain well and manage a range of long term conditions.
