General information
Service offered: Post-diagnostic support for people living with dementia, their caregivers, and families, in the form of information, advice and signposting. We make referrals to health providers to address unmet health needs, to befriending services, carer’s peer support groups and activity groups. Additionally, we help with referrals for benefits maximisation, applying for Lasting Power of Attorney and to other health and social care organisations. We have access to a dementia knowledge centre, so can help people to understand and implement coping strategies for a wide range of issues. We can carry out assessments at home, over the telephone or other convenient location.
Target group: People living with dementia, their families, and caregivers.
