Service offered: Advice, guidance and practical support for care leavers aged 18-21 (up to 25 if in full time education) and for unaccompanied minors. We make sure young people get the best possible transition into adulthood. You can get support and guidance with: your money and documents / where you live / your safety / getting your voice heard / your health / your education. Out of Hours - 020 8583 2222

Target group: The leaving care team work with 18 – 21 year olds, or up to 25 if in higher education, care leavers who require support through exiting the care system and transition to adult services. Can support Children and young people born in the UK and unaccompanied asylum seeking children who are in care, have been in care, or are leaving care.
