Service offered: Practical support for people on discharge from hospital, including assistance with claiming Attendance Allowance. Carers support volunteers work with patients and their carers facing life limiting illness, including sitting service and shopping. Support to children and families whilst a parent is undergoing chemotherapy/radiotherapy, treatment for cancer.
Target group: People being discharged from hospital, patients with life limiting illnesses and their carers.
